Forms

Use the links below to access specific forms needed for various activities. Be sure to complete all fields and contact Academic Services if you have any questions regarding their completion.

Some of these forms are writable pdf documents! To complete them electronically, save the document to your computer, type into the available fields, save, and email the document as an attachment directly to rossacadservices@umich.edu.  Otherwise, all forms should be printed, completed by hand and delivered to Academic Services.

Course/Registration Forms:

  • Independent Research Projects: Business School students may choose to do individual or group Research Projects for course credit, under guidelines outlined in the program Bulletins. Graduate level projects are numbered 750; undergraduate projects are numbered 399. These projects involve academic research on a topic coordinated with a sponsoring faculty member, and approved by the Department Chair person. Tuition will be charged for the term in which student is registered for coursework.
     
  • Drop/Add Verification Form: This form is required in order to drop or add a class after the drop/add deadline.
     
  • Foreign Language Credit Form: Graduate students taking Undergraduate level foreign language classes need to complete this form to receive credit.
     

Letter/File Request Forms:

  • Letter Request: Use this form to request letters verifying that you are a student and when you will graduate. These letters are often needed for employers or to enable non-US family members to attend your graduation. You can view templates of the common letters here:
     
    • Enrollment Verification: Indicates that you are enrolled in one of our degree programs and an estimate of when you will most likely graduate.
    • OPT Enrollment Verification: Certifies that you are a full-time student in your degree program, and gives your anticipated program completion date.
    • Commencement Invitation: Used to invite international family members to the commencement program. International students may also request letters for their international family members from the University by emailing visarequest@umich.edu. Please be aware that requests made through the University can take up to three weeks to process and students must indicate which ceremony they expect to attend.
    • Program Completion: This letter verifies that you have completed all program requirements, but have not yet had your degree conferred. Please note that we cannot issue this letter until all grades are submitted.
    • Graduate Verification: This letter verifies that you have completed your degree and that it has been officially conferred by the University.  Letters cannot be generated until the official conferral date has passed.  For April graduates, this usually occurs in early June, December graduates in early January, and August graduates in early September. 
  • File Material Request: Use this form to request copies of material in your student file.

    While students may request copies of transcripts in their student files, please be aware of the following:

    • Outside agencies requesting transcripts (e.g., employers, other grad programs) usually require official transcripts directly from the issuing institution.
    • Copies of transcripts in your file will be marked "COPY" before being released.
    • Especially for U.S. schools, students should request official copies of transcripts directly from the issuing institution to avoid these concerns. 
    • Official copies of any University of Michigan transcripts can be ordered, free of charge, via Wolverine Access.  Fees may be imposed for rush orders.
